Recommended Motion:

VI.3. Ratification of Commercial Warrants - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education ratify the commercial warrants issued from February 28, 2017 through March 31, 2017.  These warrants total $1,531,250.41.

Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education ratify the purchase orders issued between February 27, 2017 and March 31, 2017 totaling $1,308,384.03.

Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education approve the Consultant Report dated April 18, 2017.

VI.6. Approval to Enter into a Disclosure Dissemination Agent Agreement with Keygent Advisors, LLC - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends the approval of the contract with Keygent LLC to file the required "annual" and/or "material event" reports to the repositories of each bond series that have been issued by the District.

Rationale:

Each year the District is required to file an annual disclosure statement updating bond holders on the fiscal stability of the District and/or any material events occurring in the District which might influence bond holder action.

Keygent, under the terms of the agreement, will ensure that the required annual statements are submitted within the proper time frame and in the proper manner.

VI.7. Approval of Completion of Contract for Proposition 39 Building Automation System Energy Project - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education approve the Completion of Contract for the Proposition 39 Building Automation System Energy Project.  All work was performed in accordance with the contract documents and there are no outstanding issues.

Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education approve the Completion of Contract for the Proposition 39 Building Lighting Energy Project.  All work was performed in accordance with the contract documents and there are no outstanding issues.

Recommended Motion:

Karen Reed, Assistant Superintendent, Human Resources, recommends that the Board of Education approve Costco membership for the Extended Day Program Director, and six program leads, which runs from April 1, 2017 through May 31, 2018.

VI.12. Ratification of Overnight Trip for the South Pasadena High School Track Team to attend the Jim Cerveny Track Invitational in San Diego, California April 14, 2017 – April 15, 2017 – GERVAIS
Recommended Motion:

Christiane Gervais, Assistant Superintendent, Instructional Services, recommends that the Board of Education ratify the participation of twenty-four members of the South Pasadena High School Track team, two coach chaperones, C.B. Richards and Shevon Studdart, and two parent volunteers, at the Jim Cerveny Track Invitational track meet in San Diego, California, which occurred Friday, April 14, 2017 to Saturday, April 15, 2017.

VI.13. Approval of Overnight Trip for the South Pasadena High School Boys Volleyball Team to attend Santa Barbara Volleyball Tournament April 20, 2017 – April 22, 2017 – GERVAIS
Recommended Motion:
Christiane Gervais, Assistant Superintendent, Instructional Services, recommends that the Board of Education approve the participation of fourteen members of the South Pasadena High School Boys Volleyball team, two coach chaperones, Ivy Chew and Kenneth Carter, and one parent volunteer, Andrea Fox, at the Karch Kiraly Tournament of Champions volleyball tournament at Santa Barbara High School in Santa Barbara, California from Thursday, April 20, 2017 to Saturday, April 22, 2017.

VI.15. Approval of Williams/Valenzuela/CAHSEE Lawsuit Settlement Quarterly Report on Uniform Complaints - GERVAIS
Recommended Motion:

Christiane Gervais, Assistant Superintendent of Instructional Services, recommends that the Board of Education approve the Williams/Valenzuela/CAHSEE Lawsuit Settlement Quarterly Report on Uniform Complaints for the third quarter, dated from January 1 to March 31, 2017.  No complaints were registered with the Office of Instruction during this period.

VI.16. Approval of Proposal from APEX Learning to Provide Virtual Classes for Home-Hospital Teaching – LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve the price proposal from Apex Learning to provide virtual courses for Home-Hospital Teaching Support March 1, 2017 through February 28, 2018.



VI.17. Approval of Non-Public School Placement – LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve placement for a high school student to attend The Hillside School and Learning Center, a non-public school in La Crescenta, for the remainder of the 2016-2017 school year. The cost of tuition is $6,358 for the balance of this school year and will be paid from Resource Code 65000.0.


VI.18. Approval of a Non-Public Agency to Provide Speech and Language Services – LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve continued Speech and Language services for the remainder of the 2016-2017 school year for a special education student, per their Individual Education Plan (IEP). Services will be provided by Alicia A. Elliott Ed. D., Speech Pathologist, Inc., a Non-Public Agency. The cost of services is $7,440 and will be paid from Resource Code 65000.0.


VI.19. Approval of Non-Public School Placement and Parent Reimbursement for Travel - LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve placement for a high school student to attend Provo Canyon School, a residential non-public school in Utah, for the remainder of the 2016-2017 school year. The cost of tuition is $36,970 and will be paid from Resource Code 65000.0. 


VI.20. Approval of Non-Public School Placement and Parent Reimbursement for Travel - LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve placement for a high school student to attend Heritage School, a residential non-public school in Utah, for the 2016-2017 school year. The cost of tuition is $7,720 and will be paid from Resource Code 65000.0.


VI.21. Approval of Contract for Special Education Transportation Services – LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve the contract for Special Education Transportation Services for the 2017-2018 school year for special education students, per their Individualized Education Programs (IEPs). Services will be provided by Jorge’s Get Around.


VI.22. Ratification of Contract for Special Education Transportation Services – LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education ratify the contract for Special Education Transportation Services for the 2016-2017 school year for special education students. Services will be provided by Safeguard, Inc.

VII.1. Approval of Multi-Prime Contract with Balfour Beatty Construction for Pre-Construction and Construction Management Services - YANTZ
Recommended Motion:

Dr. Geoff Yantz, Superintendent, recommends Board of Education approval to enter into a CM Multi-Prime contract agreement with Balfour Beatty Construction to provide pre-construction and construction management services for the Marengo Elementary School Roof and HVAC unit replacement project.

VII.2. Approval of Resolution 2016-2017-29: Resolution Supporting the Designation of District Representatives and Authorization to File Applications for the School Facility Grant - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education adopt Resolution No. 2016-2017-29, supporting the designation of District Representatives and authorizing the filing of applications to determine School Facility Program new construction eligibility and modernization eligibility and, if eligible, applications for new construction and modernization funding with the State Allocation Board.

VII.3. Approval of Resolution 2016-2017-30: Resolution In the Matter of Applications Received When Bonding Authority is Unavailable - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education adopt Resolution No. 2016-2017-30, in the matter of applications received when bonding authority is unavailable.  This resolution is required when submitting a funding application to the State Allocation Board.

Recommended Motion:
