Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education approve the income, expense and the balance sheet statements for the South Pasadena Middle School Associated Student Body (ASB) organization, as presented in the attachment.

VI.3. Ratification of Purchase Orders - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education ratify the purchase orders issued between November 28, 2016 and December 16, 2016 totaling $260,577.73.

VI.4. Ratification of Commercial Warrants - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education ratify the commercial warrants issued from November 30, 2016 through December 16, 2016.  These warrants total $1,018,262.60

VI.5. Ratification of Phone System Maintenance and Licensing Agreement with Respond Communications Service Inc. - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education ratify the agreement with Respond Communications Plus, Inc., for the phone service maintenance and software licensing.  The renewal extends from December 6, 2016 through December 5, 2017 at a cost to the District of $9,073.

VI.6. Approval of Easy Turf Completion of Contract for the Tiger Patio Phase II Project at South Pasadena High School - LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education approve the Completion of Contract for the renovation of the South Pasadena High School Tiger Patio, Phase II.

Rationale:
All work was performed in accordance with the contract documents and there are no outstanding issues.

VI.7. Approval of Easy Turf Completion of Contract for the Tiger Patio Phase III Project at South Pasadena High School- LUBS
Recommended Motion:

Dave Lubs, Assistant Superintendent of Business Services, recommends that the Board of Education approve the Completion of Contract for the recently refurbished Tiger Patio area in between the two-story classroom Bldg. 400 and Bldg. 500 at South Pasadena High School.

Rationale:
All work was performed in accordance with the contract documents and there are no outstanding issues.

VI.12. Approval of Field Trip for South Pasadena High School Wrestling Team to attend Morro Bay High School Wrestling Invitational, January 20-22, 2017 - GERVAIS
Recommended Motion:
Christiane Gervais, Assistant Superintendent of Instructional Services, recommends that the Board of Education approve the field trip request for twenty-four students and two teacher chaperones (Varsity Wrestling Coach Al Shuton and Junior Varsity Wrestling Coach Victor Gonzalez) to attend the Morro Bay High School Wrestling Invitational, January 20-22, 2017.

VI.13. Approval of Overnight Trip for South Pasadena High School students to attend Yosemite Institute in Yosemite National Park May 21, 2017 - May 26, 2017 - GERVAIS
Recommended Motion:
Christiane Gervais, Assistant Superintendent, Instructional Services, recommends that the Board of Education approve the overnight trip request for sixty SPHS students and five teacher chaperones, Mr. Casey Shotwell, Mr. Josh Whitney, Mr. Don Wielenga, Mr. Shane Mills, and Ms. Trang Huynh, to attend the Yosemite Institute in Yosemite National Park May 21, 2017 to May 26, 2017.

VI.14. Approval of Williams Quarterly Report - GERVAIS
Recommended Motion:
Christiane Gervais, Assistant Superintendent, Instructional Services, recommends that the Board of Education approve the Williams/Valenzuela/CAHSEE Lawsuit settlement Quarterly Report on Uniform Complaints for the second quarter dated from October 1, 2016 to December 31, 2016.  No complaints were registered with the Office of Instruction during this period.

VI.15. Approval of Non-Public School Placement – LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve placement for an elementary school student to attend the Marianne Frostig Center of Educational Therapy, a non-public school in Pasadena, for the remainder of the 2016-2017 school year. The cost of tuition is $16,717 and will be paid from Resource Code 65000.0.


VI.16. Approval of Non-Public School Placement – LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve continued placement for an elementary school student to attend Tobinworld, a non-public school in Glendale, for the 2016-2017 school year, including ESY 2017. The cost of tuition is $47,165, and will be paid from Resource Code 65000.0.


VI.17. Approval of a Non-Public Agency to Provide Behavior Services – LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve one-on-one support services for the 2016-2017 school year for a special education student, per their Individual Education Plan (IEP). Services will be provided by Inizio Interventions, Inc., a Non-Public Agency. The cost of services is $39,702 and will be paid from Resource Code 65000.0.


VI.18. Approval of Non-Public School Placement and Parent Reimbursement for Travel - LEFEVRE
Recommended Motion:

Dr. Dennis Lefevre, Executive Director of Student Support Services, recommends that the Board of Education approve the amended total for the previously approved non-public school continued placement in the amount of $140,998.



Rationale:

The Provo Canyon Master Contract was Board approved on October 18, 2016. The contract amount totaled $140,998, however, the recommendation stated $101,470. The recommendation is being updated to match the original contract and there are no additional costs from the original contract.

Recommended Motion:
Dr. Geoff Yantz, Superintendent, recommends that the Board of Education approve Resolution 2016-2017-17 - Resolution Authorizing the Issuance of the South Pasadena Unified School District Election of 2016 General Obligation Bonds Series A Pursuant to Certain Provisions of the Government Code in a Principal Amount Not to Exceed $25,000,000, and Approving Certain Other Matters Related Thereto.

VII.2. Appointment of Members to the Measure SP Citizens' Bond Oversight Committee - YANTZ
Recommended Motion:

Dr. Geoff Yantz, Superintendent, recommends that the Board of Education appoint the following members to the Measure SP Citizens' Bond Oversight Committee: Traci Fleming, Jennifer Kowal, Jon “Scott” Kuhn, Joseph Loo, Scott Moe, Steven Norris, and Dean Serwin.

VII.3. Approval of Design to Construct Math Classrooms at South Pasadena High School and Submit Plans to Obtain Division of State Architect Approval for Construction - YANTZ
Recommended Motion:

Dr. Geoff Yantz, Superintendent, recommends that the Board of Education approve plans for the construction of six classrooms, a flexible lab, conference room and softball locker room at South Pasadena High School and submit plans to obtain DSA approval for construction.
